How to Apply for the A+ Fit School Designation

1. Download and read all designation application documents.
   • A+ Fit School Designation and Mini-Grant RFP
   • Designation Application
   • A+ Fit School Self-Assessment Instrument
   • Designation Endorsement Form
2. Assemble a diverse team of school staff members and work together to complete the self assessment instrument and the designation application.
3. Obtain official endorsement from the school principal or lead administrator and the district superintendent (must use A+ Fit School Designation Endorsement Form).
4. Email a copy of the designation application to hwtfc.grantapps@healthwellnc.com AND mail two hard copies of the application and all supporting materials to:

   Greenville, NC 27858

The electronic submission is due by 5:00 pm on February 24, 2010 and the postmark on the hard copy submission should be no later than February 24, 2010.
